Healthcare professionals requesting clinical records
Other Health NZ regions and hospitals requiring information for ongoing acute care, can send us an email with your request.
Include on the email:
your name
your title
your fax number
the patient's NHI
the patient's first and last name
the patient's date of birth
details of exactly what information you require, for example discharge summary from operation, clinic letter from rheumatology, imaging result from angiogram
for what date or date range you want the information for.
Detailed information will help speed up our response to your request for information.
Alternatively you can call us if you want to speak to one of our team to clarify any points.
Requesting healthcare records for yourself or a dependant
Information from your own record or the record of a dependant can be requested from the Clinical records department. This will include documentation from both North Shore Hospital and Waitakere Hospital.
Complete the request form and and provide the requested identification depending on who's information you are requesting. Identification requirements are on page 3 of the form.
After you have completed the form you can email it, bring it in, or post it to us.
Requesting any other person's record
If the request is for the clinical record of someone who is not your dependant (16 years or under) then that person's consent is required in writing. This includes your spouse, parents, adult children, brothers or sisters, as well as anyone else who is not your dependent child.
