Diabetes
Diabetes is a long term condition due to either autoimmune based loss of insulin secretion as in Type 1 Diabetes or a combination of loss of insulin secretion coupled with insulin resistance in the more common Type 2 Diabetes.
Description
- Paediatric Services - working closely with children, youth and their families - Paediatricians, Clinical Nurse Specialist and Dietitian
- High Risk Foot Assessment Clinic - Held each Thursday morning - Podiatrist
- Diabetes and Pregnancy - Focusing on pregnant women with either Type 1 Diabetes, Type 2 or Gestational Diabetes - Diabetes Physician, Clinical Nurse Specialist and Dietitian work collaboratively with the pregnant woman, their Obstetrician and Midwife to optimise glycaemic control.
- Adult Diabetes Service - Accept referrals from General Practitioners, Nurse Practitioners and Hospital Physicians to assist people with diabetes to improve their glycaemic control - Physician, Clinical Nurse Specialist, Dietitian, Registered Nurse.
- Clinical Nurse Specialist Service - Provide education and support to people living with diabetes, General Practitioners, Nurse Practitioners, Practice Nurses, Secondary Services staff and other health providers. Education and support to people living with diabetes can be on a one to one basis or as part of a group.
- Dietitian Service - Provides support to people living with diabetes and their families. Also provides support for people at risk of developing diabetes - Dietitians working in the hospital and community.

People with Diabetes can self-refer to the Community Based Clinical Nurse Specialist as appropriate.
Your GP or Nurse Practitioner can refer to the Hospital based Physician or Paediatric clinics.
